First appointment: What you should bring with you
Ideally, you should come wearing athletic and comfortable clothing. You should remember to get a prescription from your doctor if your insurance plan requires one (All Premera Blue Cross patients are required to have one). If your are using insurance benefits, be sure to bring along your Member ID card.
Does your Health Insurance cover Payment of costs: We are happy to assist you
If you have any questions about whether your health insurance covers the costs of your treatments, we'd be happy to assist you - just call us at 360-260-9999 or come into our studio.
Office Policies
We require 24 hours advance notice for cancellation of sessions. For sessions scheduled with less than 24 hours notice we require 4 hours advance notice of cancellation by phone at (360) 260-9999.
Sessions scheduled within 4 hours of appointment time are non-refundable. Cancellations failing to meet these requirements will be charged in full.
Payment can be made by cash, check or credit card. We accept Visa and MasterCard. Checks should be made out to Full Moon Massage. Accounts past due are subject to finance charges. Payment is due at
time of service.
No-shows are inconvenient for everyone. Therefore, our office has institiued a $20 no-show fee. After-hour messages regarding cancellations may be left at 503-913-1317. Thank you for your
